NEW PATIENT PAPERWORK

We request you arrive fifteen minutes early in order to allow yourself ample time for completing our new patient paperwork.  Prefer to get a head start?  Feel free to print out the three documents below and fill them out before you come in to the office. 

Due to HIPAA regulations and to protect our patient's privacy, we are unable to accept electronic transmissions of the completed forms (i.e. - e-mail, fax, etc.).  Not to worry, if you are not able to get a head start, you will find the cozy chairs in our reception are a comfortable place to relax with a clipboard and jot down the info we will need before you see the doctor.

MESSAGE OF THE WEEK

Patients in our practice receive an inspiring Message of the Week when they visit. Many report back to us that they enjoy sharing them with their loved ones and colleagues.
